To be eligible for Happy Hearts, you must meet all of the requirements below:
- You must have Original Medicare Parts A and B, with Medicare as your primary payer. This means you enrolled directly with Medicare, not through an insurance company.
- You cannot have Medicare Advantage, PACE (Program of All-Inclusive Care for the Elderly), or be enrolled in hospice.
- You cannot already be aligned with another ACCESS participant for the same health track within the prior 90 days. If you were recently enrolled with another ACCESS program, you may need to wait before switching.
- You must meet the clinical criteria for one of Happy Hearts' available tracks. The registration website will show you which conditions qualify for each track. Happy Hearts has a program for:
- Enhanced Chronic Kidney Management (eCKM)
Are you in the control group?
Some Medicare beneficiaries have been randomly assigned to a comparison group for evaluation purposes. If you're in the control group, you cannot enroll for 12 months. This is not based on anything about you or your health. Your Medicare benefits and coverage are not affected.
What happens during enrollment?
When you visit the Happy Hearts registration website, the system will verify your eligibility by checking your information against Medicare records through the CMS ACCESS system. The website will let you know if you're approved or if there's a reason you can't enroll.
