I need help enrolling in the Happy Hearts program

Need help enrolling? No problem! Happy Hearts enrollment happens through our web-based registration process.

Here's how to enroll:

  1. Visit the Happy Hearts registration website on your computer or mobile device web browser.
  2. Tap or click "Get Started" or "Enroll Now."
  3. The website will ask you to provide your information so we can check your eligibility:
    • First Name/Last Name must match your Medicare card or official ID, including hyphens (Smith-Johnson), apostrophes (O'Connor), and suffixes (Jr/Sr).
    • Date of birth must match your Medicare records.
    • Your phone number will be your username for logging into the app later.
  4. Select the health track that applies to you. The website will explain the different options.
  5. The system will verify your eligibility by checking your information against Medicare records through the CMS ACCESS system. This may take a few minutes.
  6. If your information matches your Medicare records and you meet the clinical criteria for the track, you'll be approved.
  7. A verification code will be sent to your phone number via text message. Enter this code on the registration website to confirm your identity.
  8. Once verified, you'll be directed to download the Happy Hearts app:
    • iPhone: Search the Apple App Store
    • Android phone: Search the Google Play Store
  9. Open the app, log in using your phone number, and enter the verification code sent you via text message.
  10. Complete the welcome questions in the app to tell us more about your health.
  11. Once the welcome screens are completed, an order for a blood pressure monitor will be placed automatically.

That's it! You're now enrolled in Happy Hearts and ready to get started.
