How do I enroll in the Happy Hearts program?

We really appreciate your interest in Happy Hearts! Enrollment is at no cost to you and happens through our web-based registration process.

Here's how to get started:

  1. Visit the Happy Hearts registration website on your computer or mobile device web browser.
  2. Tap on "Get Started" or "Enroll Now."
  3. On the eligibility screen, enter your information exactly as it appears on your Medicare card:
    • First Name/Last Name must match your Medicare card or official ID, including hyphens, apostrophes, and suffixes (Jr/Sr). This is how your eligibility will be verified.
    • Date of birth must match your Medicare records.
    • Your phone number will be your username when you log into the app later.
  4. Select the health track that applies to you. The website will explain what each track includes and which conditions qualify.
  5. The system will check your eligibility by verifying your information against Medicare records through the CMS ACCESS system. This may take a few minutes.
  6. If your information matches Medicare records and you meet the clinical criteria, you'll be approved and move to the next step.
  7. A verification code will be sent to your phone number via text message. Enter this code on the website to confirm your identity.
  8. Once verified, you'll be directed to download the Happy Hearts app from the Apple App Store or Google Play Store.
  9. Open the app and log in using your phone number and the verification code we sent you during registration.
  10. Answer welcome questions in the app about your health and what brought you to Happy Hearts.
  11. If you're approved for a blood pressure monitor, an order will be placed automatically and you'll receive tracking information.

That's it! You're now enrolled in Happy Hearts and ready to start tracking your health.

Important to know:

Once you enroll, you're committed to the program for a 90-day period. During this time, you cannot switch to another ACCESS program. After 90 days, you can request to end your participation with Happy Hearts anytime, and you'll have other options available.
